Daly City Partnership: Program Coordinator

Daly City is seeking a Program Coordinator focused on senior wellness for the Healthy Aging Response Team, a department of Daly City Partnership. Responsibilities for this role will include assisting the Director in implementing the PEARLS evidence-based program, empowering older adults with depression to enhance their overall well-being. The role also requires providing support and training to staff and volunteers who will serve as coaches, providing 1-1 sessions over a 4-5-month period, and developing problem-solving techniques.

Location: This position is 100% on-site at the Daly City Community Service Center.

MINIMUM QUALIFICATIONS

  • Master’s degree from an accredited college or university with major course work in a related field, such as Geriatrics, Health, Human Services, Sociology, Psychology, Social Work, or Public Relations

    • Registered Associate Marriage and Family Therapists (AMFT) preferred 

  • Demonstrated proficiency in Microsoft Suite (Word, Excel, PowerPoint)  

  • High degree of accuracy, efficiency, and attention to detail   

  • Maintain proper professional boundaries and confidentiality

  • Willingness and ability to ask for help and to work under supervision

  • Must demonstrate cultural humility, treat all people with respect and believe that different experiences and perspectives add value. 

  • 3-5 years supervisory experience in a non-profit, community organization or health related field

responsibilities and tasks

  • Direct day-to-day Coordination of HART office site and HART programming (classes, home visits, phone line, drop-in clients, meal and grocery delivery, outreach, etc.)

  • Support the Associate Director and Director in buildout of new PEARLS programming, including outreach, engagement, volunteer and staff recruitment process, and creation of high-standards/high-support culture

  • Provide Mentorship, act as team lead and support wellness coaches, volunteers and interns in small group and 1-1 training and supervision meetings 

  • Train and supervise wellness coaches as well as 1 case manager, ensuring that they are well-equipped to fulfill their roles, have a clear understanding of the program(s), client interaction standards, data entry, etc.

  • Ensure that the case managers and wellness coaches comply with all relevant regulations, policies, and guidelines, which can be complex and subject to change

  • Serve as an excellent representative of the Partnership to internal and external stakeholders at regular meetings, such as committees, boards, government entities and collaborative partnerships 

  • Proactively attend all mandatory training and attend at least 2 training opportunities quarterly that will contribute to the team’s success

  • Identify areas of challenges and success and seek opportunities for growth in partnership with your direct supervisor at semi-monthly 1-1 supervision meetings 

  • Identify areas of growth and program improvement and work alongside direct supervisor to create and implement effective solutions

  • Demonstrate fiscal responsibility through efficient allocation of supplies, resources, and implementation of programming

  • Conduct occasional outreach regarding HART program services at community venues

  • Assist in evaluation of services and narrative reports on outcomes by providing client stories and feedback to supervisor for reports and publications

  • Input accurate data entry into client relations management systems, as well as other tracking systems throughout the workday while processing client, volunteer and staff requests

  • Other duties as assigned

Compensation and Benefits

  • Full-time position, 40 hrs/week.  Salary Range: $ 31-33 DOE
