Daly City Partnership: Case Manager I/II

Daly City is seeking a Case Manager. As a Case Manager I/II for the Healthy Aging Response Team, a department of Daly City Partnership, you will be an integral part of the team supporting older adults (seniors 50+) and disabled adults. You will assess, triage and support clients with a wide array of information, referrals and services. You will provide in-home health assessments, 1-1 coaching, care coordination, application assistance, referrals and other services related to social determinants of health. The case manager will be part of a multi-disciplinary team of staff and volunteers who also provide support via phone and drop-in services.

Location: This position is 100% on-site will be located at the Daly City Community Service Center.

MINIMUM QUALIFICATIONS

  • 2 to 4 years of case management experience  

  • Demonstrated proficiency in Microsoft Suite (Word, Excel, PowerPoint)

  • High degree of accuracy, efficiency, and attention to detail   

  • Maintain proper professional boundaries and confidentiality

  • Willingness and ability to ask for help and to work under supervision

  • Must demonstrate cultural humility, treat all people with respect and believe that different experiences and perspectives add value. 

  • Associate or bachelor’s degree in social work, psychology, sociology, related field or equivalent work experience.

  • Valid driver's license

  • Bilingual in one of the following languages is required for this position: 

    • Spanish, Cantonese, or Mandarin

  • Knowledge of:

    • Resources including local public and non-profit organizations, including senior and disabled adult services

    • Basic health screening and assessment, the principles and accepted norms of older and disabled adult care, mental health needs, and familiarity with issues facing seniors and disabled adults.

responsibilities and tasks

  • Proactively attend all mandatory training and attend at least 2 training opportunities quarterly that will contribute to the team’s success

  • Identify areas of challenges and success and seek opportunities for growth in partnership with your direct supervisor at semi-monthly 1-1 supervision meetings 

  • Participate in collaborative team meetings and trainings and utilize what you have learned to benefit clients

  • Conduct regular home visits to homebound seniors and disabled adults, carry out assessments and provide follow-up communication at regular intervals for continued short to mid-term needs, supporting clients with their stabilization plan

  • Provide wrap-around services, referrals and warm hand-offs to support clients in the application process for qualifying social services, such as home-delivered meals and groceries, technology classes, reduced cost utilities, mental health support, recreation, Medical/Medicare, Social Security, In Home Support Services, Cal Fresh, low-income senior housing search, etc.

  • Input accurate data entry into client relations management systems, as well as other tracking systems throughout the workday while processing client requests.

  • Support client application process for seasonal community service projects (holiday food, Family Resource Day, etc.)

  • Conduct occasional outreach regarding HART program services at community venues.

  • Assist in evaluation of services and narrative reports on outcomes by providing client stories and feedback to supervisor for reports and publications

Physical Requirements

  • Stand and sit and move around for extended periods of time

  • Lift up to 50 lbs of weight

Compensation

  • Full-time position, 40 hrs/week. 

  • Salary Range: $ 29-31 DOE (Determination of Case Manager I or II DOE)
